When you book your initial appointment with Chiaf Chiropractic Clinic, we will send you a link to complete your new patient paperwork online. If you prefer, you may come in a few minutes early to fill in the paperwork in our office. Also, please bring your ID and insurance card.
PATIENT REGISTRATION

Your First Visit

When you arrive, our bubbly front desk staff will greet you by name, help you feel welcomed, and answer any of your questions. They’ll collect your insurance information and verify your coverage for you.

Then you’ll sit down with Dr. Chiaf for an in-depth chat about your health and what has brought you to our office. He’ll take the time to truly listen, so he can understand how your symptoms affect your quality of life. Next, he’ll conduct a thorough examination, which will include X-rays (we don’t X-ray infants or young children).

Your Second Visit

We’ll schedule your follow-up for the next day or the day after, which gives Dr. Chiaf time to thoroughly study and analyze your exam results and your X-rays, so he can diagnose the underlying cause of your condition and develop an appropriate care plan.

When you return for your second visit, Dr. Chiaf will explain all his findings and answer all your questions. He’ll lay out his recommendations for care, along with all the costs involved and your insurance coverage (if you’re using insurance).

If you’re ready to begin, you’ll receive your first adjustment at this visit.
